🧱 The First Principles of Gout
- Gout is a crystal arthropathy, linked to hyperuricaemia, leading to deposition of monosodium urate crystals within the joints and soft tissues, leading to acute inflammatory arthritis.
- Hyperuricaemia may result from decreased renal clearance (kidney disease) or increased production of uric acid (purine-rich diet or cell turnover disorders).
- Joint inflammation results in sudden pain, redness, warmth, and swelling.
- Without long-term urate lowering therapy, recurrent gout attacks cause chronic joint damage, large crystal deposits (tophi), and erosive arthritis.
