🧱 The First Principles of Gout

  1. Gout is a crystal arthropathy, linked to hyperuricaemia, leading to deposition of monosodium urate crystals within the joints and soft tissues, leading to acute inflammatory arthritis.
  2. Hyperuricaemia may result from decreased renal clearance (kidney disease) or increased production of uric acid (purine-rich diet or cell turnover disorders).
  3. Joint inflammation results in sudden pain, redness, warmth, and swelling.
  4. Without long-term urate lowering therapy, recurrent gout attacks cause chronic joint damage, large crystal deposits (tophi), and erosive arthritis.
